Notice on Protection on Medicare Health Information

To protect the privacy of Medicare beneficiaries and to comply with the requirements of HIPAA and other federal laws, protected health information will not be disclosed in telephone or written inquiries unless the medical office supplies all of the following elements at the time the request is made: provider name; provider number; beneficiary’s first and last name; Medicare number; and either the date of birth (for eligibility information) or the date of service (for claim information).
